Pathways is a community-based, not-for-profit organization with one mission: to deliver compassionate, high quality health patient and family-centered home health and hospice care that promotes comfort, independence and dignity. Committed to serving the diverse Bay Area community, we have the knowledge and staffing experience to assist your loved one whether they are moving home or to a health care facility.

Last year, Pathways Home Health, Hospice & Private Duty cared for nearly 5,000 home care and hospice patients ranging in age from newborn to 100. Your initial conversation with Pathways will be with an admission nurse who is supported by a team of patient care services. These services include nursing, home health aide, palliative care, hospice medical directors, social work, pharmacy, bereavement, spiritual care, integrative therapies, and an after hours “dream team.”

A nurse is on-call 24 hours a day for telephone consultation or visits if needed.
