CHW’s network of care centers offers a range of medical resources, including urgent care, ambulatory surgery centers, imaging centers, home care, hospice, and laboratory services. Not everyone lives near a major hospital, so CHW is making health care more accessible by bringing the right resources closer to where you live and work. In urban and rural communities alike, residents of all ages and backgrounds have access to primary care, preventive treatment, clinical support, chronic disease management, and a host of therapeutic specializations. And we give you more than one way to search for the care center that best fits you and your family’s needs.

Don’t know what kind of care center you need? Here are some helpful definitions:

Urgent Care – This is a care center that provides medical care for conditions requiring attention but that aren't an immediate, serious threat to life or health.

Ambulatory/Surgery – These centers are considered outpatient centers because you won’t need to spend the night in the hospital following your treatment, which may include surgery, ophthalmology, pain management, etc.

Home Health – Our home health agencies provide comprehensive care for patients who have recently been discharged from the hospital and/or homebound individuals who need short-term, skilled nursing or therapeutic care. Includes Hospice services.

Imaging – These centers will provide services such as X-ray, mammography, nuclear medicine, ultrasound, CT, MRI and PET.

Laboratory - Provides patient services such as blood draw and specimen collection stations.

Specialty - All other services that do not fall into one of the above categories.  Some examples are Cancer, Women's Care and even Wound Healing Centers.
