Community Grants Application
The following is the timeline and materials you will need to apply for a CHW Community Grant.

Step One: Send us a completed Letter of Intent.

Step Two: You will be notified if you have been selected to submit a grant proposal.

Step Three: If we’ve encouraged you to apply for the grant, submit a grant proposal.

Step Four: You will be notified if you have been selected to receive the grant funds.
Timeline
Intent to Apply Due: July 27, 2011
Invitation to Submit Proposal/Declination Notification Sent: August 24, 2011
Grant Proposal Due: September 16, 2011
Final Grant Approval by CHW Investment Committee: November 29, 2011
