Care for Chronic Conditions
For those living with chronic disease, support services are often the missing link between the physician’s office, the hospital, and the home. CHW has taken a bold and strategic approach to chronic disease management, developing programs at our medical facilities that address chronic disease in the community, with a specific emphasis on those who are poor and vulnerable.
Many CHW hospitals offer disease management programs, and all provide disease-specific discharge education for patients as a standard of quality care. Chronic disease self-management programs ultimately result in reduced health care expenditures, improved health outcomes and fewer unnecessary hospital visits.
Patients who participate in our chronic disease self-management programs are also able to better maintain many of their health and behavioral improvements over time, leading to significant, measurable progress in patient outcomes and quality of life.
